Job Summary

Key Responsibilities
  • Interpret physician referrals and conduct comprehensive evaluations to identify patient needs.
  • Develop and implement individualized treatment plans to achieve functional goals and improve patient outcomes.
  • Monitor and evaluate patient progress, adjusting treatment plans as needed to ensure optimal care.
  • Collaborate with healthcare teams to provide seamless patient care and achieve desired outcomes.
  • Maintain accurate and detailed documentation of patient care and progress, adhering to departmental policies and accreditation standards.
  • Stay up-to-date with industry developments and best practices through ongoing education and professional development.
Requirements
  • Doctorate degree in Physical Therapy from an accredited institution.
  • Physical Therapy License in the State of Illinois or Wisconsin.
  • Basic Life Support (BLS) certification.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
