About the Job

Athena Health Care Systems is seeking a skilled Licensed Practical Nurse (LPN) to join our team at Bayview Health Care in Waterford, CT. As a LPN, you will play a vital role in providing direct nursing care to residents and supervising the day-to-day nursing activities performed by nursing assistants.

Responsibilities
  • Meet the nursing and rehabilitation needs of each resident
  • Maintain current resident care plans on all residents under your care
  • Follow physician's orders and render complete nursing care, treatments, dressings, and other physician orders
  • Transcribe physician's orders promptly and accurately
  • Review patient records daily to assure accuracy and completeness
  • Document comprehensive and complete nursing notes
  • Visit each resident daily to evaluate resident needs
  • Maintain accurate awareness of residents on alert list
  • Document and report any unusual or significant findings; contact the physician when necessary
  • Administer medication according to procedure
  • Maintain an accurate and safe control of all drugs for which you are responsible
  • Initiate any and all emergency procedures as necessary
  • Demonstrate correct safety techniques
  • Contribute to and promote positive resident and family relations
  • Comply with facility Privacy Practices Policies & Procedures, protecting residents' individual health information
Requirements
  • Must be a graduate of an approved LPN/LVN program and hold a current LPN license with the state
  • Must be CPR-certified
  • A minimum one (1) year experience in a hospital, long-term care facility, or other related healthcare facility is preferred
