Unit Information:
The LPN will work in a Long-Term Care/SNF setting, primarily focusing on memory care, dementia, and Alzheimer's patients. The facility features secured units for memory care and offers a range of services.
Responsibilities:
- Provide nursing care in accordance with state regulations.
- Collaborate with a multidisciplinary team to ensure comprehensive patient care.
- Maintain accurate patient records and documentation using the PCC charting system.
Qualifications:
- Must possess a valid LPN license.
- Experience in dementia care is highly preferred.
- Ability to work independently and as part of a team.
Work Environment:
- The LPN will be part of a supportive team, with RN oversight available.
