We are seeking a skilled Master Social Worker to join our team at Innovative Renal Care. As a Social Worker, you will play a vital role in helping patients understand their rights and responsibilities, and guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
Responsibilities- Assess patients' needs and develop personalized care plans
- Collaborate with patients, families, and healthcare teams to ensure comprehensive care
- Provide education and support to patients and their loved ones
- Meet with patients to discuss their progress and make adjustments to care plans as needed
- Document patient interactions and progress in a timely and accurate manner
- Master's degree from an accredited school of social work (MSW)
- Current state licensure (e.g. LCSW, LMSW)
- Two years of clinical work experience, preferably in an ESRD setting
- Excellent communication and interpersonal skills
