About the Role
Rio Grande Regional Hospital is seeking a skilled Registered Nurse Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring that patients receive the highest quality care and support throughout their healthcare journey.
Responsibilities
- Act as a liaison between patients, families, and healthcare providers to coordinate care and services.
- Assess patients' needs and develop individualized care plans to promote optimal health outcomes.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care and prevent readmissions.
- Provide education and support to patients and families to empower them to make informed decisions about their care.
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements
- Associate Degree in Nursing, required
- Bachelor's Degree in Nursing, preferred
- Case Management certification, preferred
- Current Texas licensure with State Board of Nurse Examiners as an RN
- 1+ years experience in healthcare
