About the Role

This LPN position involves providing high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.

The ideal candidate will have excellent communication and interpersonal skills, be able to work independently, and possess a strong commitment to delivering patient-centered care.

Responsibilities
  • Assist registered nurses in planning and delivering nursing care, promoting patient independence and rehabilitation.
  • Develop and implement individualized care plans based on patient needs and goals.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Maintain accurate records of patient interactions, treatment plans, and progress notes.
Requirements
  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
  • Current Washington State LPN license upon hire.
  • National Provider BLS certification within 30 days of employment.
  • Ability to drive and provide proof of driver's license and auto insurance upon request.
Preferred Qualifications
  • Recent experience in hospital, nursing home, or home health settings.
Compensation and Benefits

As a valued member of our team, you can expect a competitive salary range of $62,000 - $80,000 per year, depending on experience.
