LHC Group is seeking a skilled Physical Therapist to join our team in The Villages, Florida. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the Role:We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to our patients in their homes. As a Physical Therapist with LHC Group, you will be responsible for assessing and evaluating patient care needs, developing and implementing treatment plans, and providing ongoing care and support to patients and their families.
Responsibilities:- Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement treatment plans to improve and enhance patient well-being.
- Provide ongoing care and support to patients and their families, including education and training on self-care and management of chronic conditions.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care.
- Document patient progress and outcomes in accordance with LHC Group's policies and procedures.
- Current Physical Therapy licensure in the state of Florida.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
