About the Role:

As a RN Hospice Nurse with Masonicare, you'll be responsible for developing, implementing, and evaluating individualized plans of care for patients. This will involve coordinating with other healthcare professionals, identifying areas for improvement, and making recommendations for additional services as needed.

You'll also provide education, supervision, and counseling to patients and their families on home nursing procedures and other care needs. Additionally, you'll assure that patients and their families are involved in the development of their care plan whenever possible.

Requirements:

  • Current RN License required
  • Hospice and/or Home Care experience preferred, but not required
