We are seeking a dedicated and accomplished RN Hospice Specialty Registered Nurse to join our team at Masonicare Home Health & Hospice in the Branford, CT area.

Job Description

The RN Hospice Nurse ensures quality and safe delivery of hospice nursing services to patients in their homes or assisted living settings. This involves developing, implementing, and evaluating plans of care, making necessary revisions to address problems, and coordinating care with other healthcare professionals.

Hospice Nurses work closely with interdisciplinary teams to identify care needs and make recommendations for additional home health care services. They supervise client care rendered by Licensed Practical Nurses and Home Health Aides, conduct supervisory visits, and provide education and supervision to clients and their families.

The ideal candidate will have experience in hospice and/or home care, with a strong background in patient assessment, care planning, and documentation. Proficiency in computer documentation and OASIS data set creation is also required.

Required Skills and Qualifications
  • RN licensure in the state of Connecticut
  • Hospice and/or home care experience preferred
  • New graduates will not be considered
