Job Description:

The LPN/LVN will provide skilled nursing care to patients in their homes, focusing on preventative and rehabilitative measures to promote optimal health outcomes. Key responsibilities include:

  • Administering medications and treatments as prescribed by physicians
  • Monitoring patients' vital signs and reporting any concerns to the RN or MD
  • Assisting patients with daily living activities, such as bathing and dressing
  • Collaborating with other healthcare professionals to develop and implement comprehensive care plans

In addition to providing high-quality patient care, the LPN/LVN will also be responsible for maintaining accurate and detailed records of patient interactions and interventions. This includes documenting observations, treatments, and any notable changes in patient status.
