Job Summary

Phoenix Senior Living is seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Resident Care Specialist. As an integral part of our healthcare team, you will be responsible for providing high-quality care to our residents while working closely with the Wellness Director to ensure seamless execution of monthly wellness visits and Personalized Service Plans.

About the Role

This role requires a licensed practical nurse with a minimum of one year of experience working with elderly or disabled individuals. The ideal candidate should possess strong leadership capabilities, knowledge of Assisted Living industry standards, and Dementia care best practices. Key Responsibilities
  • Identify resident needs through comprehensive assessment processes
  • Ensure accurate documentation of changes in clinical care needs

  • Provide direct resident care as needed
Requirements

To be successful in this role, you must have:

  • A valid LPN license in the state of employment
  • A minimum of one year of experience working with elderly or disabled individuals
  • Demonstrated leadership capabilities and knowledge of Assisted Living industry standards
Compensation and Benefits

The estimated annual salary for this position is $62,000 - $72,000, depending on experience and qualifications. Our comprehensive benefits package includes health insurance, retirement plans, and opportunities for professional growth and development.
