Hospice Care Specialist

We are seeking a skilled Hospice Social Worker to join our team at MaineHealth. As a Hospice Social Worker, you will provide high-quality, patient-centered care to individuals and their families in the comfort of their own homes.
Key Responsibilities- Assess the social and emotional needs of patients and their families, and develop individualized care plans to address these needs.
- Provide counseling and support to patients and their families, helping them to cope with the challenges of hospice care.
- Collaborate with other healthcare professionals to ensure comprehensive, coordinated care.
- Document patient progress and outcomes, and participate in quality improvement initiatives.
- Develop and maintain relationships with community resources and organizations to support patients and their families.
- Master's degree in Social Work (MSW) from an accredited program.
- Current licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) in the state of Maine.
- Minimum of one year of experience in hospice or palliative care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong assessment and problem-solving skills, with the ability to develop and implement effective care plans.
