Primary Care Nurse Practitioner Position

As a Primary Care Nurse Practitioner at Brockton Neighborhood Health Center, you will provide high-quality medical care to an assigned panel of patients. Your primary responsibility will be to conduct comprehensive primary care services, including well examinations, immunizations, and episodic sick visits. You will also be responsible for assessing, educating, and managing patients' physical, emotional, and social needs. Additionally, you will design a plan of care based on assessment and collaborate with the patient and other healthcare professionals as needed.
Key Responsibilities:
• Conduct comprehensive primary care services, including well examinations, immunizations, and episodic sick visits.
• Assess, educate, and manage patients' physical, emotional, and social needs.
• Design a plan of care based on assessment and collaborate with the patient and other healthcare professionals as needed.
• Incorporate case management activities into practice and collaborate with the multidisciplinary healthcare team to provide continuity of care for patients.
• Initiate follow-up and specialty referrals for patients needing further work-up or monitoring of a current situation.
Requirements:
• Current Massachusetts licensure for RN in expanded role.
• Certification eligible as Primary Care Practitioner.
• Federal and State licensure for prescriptive writing privileges.
• Ability to develop and work with a multi-disciplinary team, including mid-levels, social workers, and case managers.
• Fluency in Spanish, Portuguese, French, or Haitian Creole is highly desirable.
• Previous experience in ambulatory care and/or public health preferred.
Working Conditions:
• Works in a busy community health center in a downtown business district.
• Occasional long hours may be required.
• Potential exposure to patient blood and body fluids.
Physical Requirements:
• Physical demands requiring lifting of small pieces of equipment, boxes, and/or patients not to exceed 50 pounds.
• Mostly standing and walking required.
• Visual acuity sufficient for frequent reading.
• Hearing acuity sufficient for holding conversation with or without audio devices.
Travel, Work Hours:
• Occasional statewide or national travel possible for CME/conferences.
• Includes evening and weekend hours.
• BNHC is committed to providing a safe and healthy environment for patients and staff. As a condition of employment, employees, to include new hires, are required to be fully vaccinated to include the most recent COVID 19 booster and annual flu vaccines, with the exception of those who have documented medical or religious exemptions, that must be approved by BNHC according to its protocols. Additional vaccines may be required based on the recommendation of the CDC. New hires who are not yet vaccinated for flu or COVID can arrange to be vaccinated on site.
