Position Location: McRae, GA; Serving Wheeler and Treutlen Counties

Key Responsibilities:

  • Delivering a wide array of social services to individuals and families within the educational settings of Dublin and Laurens County.
  • Facilitating attendance at scheduled medical appointments and ensuring adherence to prescribed medication regimens.
  • Maintaining regular communication with consumers' families, educators, and school counselors, documenting interactions and reporting findings to supervisors and medical directors.
  • Timely completion of progress notes within 24 hours of service delivery, ensuring documentation is scanned and available in the electronic medical record (EMR) within a week.
  • Achieving an average of 5.8 billable hours per day, monitored through daily productivity metrics with a minimum performance standard of 65%.
  • Collaborating with administrative staff to develop and maintain a weekly schedule that meets monthly contact requirements.
  • Engaging with team members to discuss progress towards established objectives, providing constructive feedback, and implementing corrective measures as necessary.

This role necessitates regular travel to clients' schools and residences.

Qualifications:

  • A High School Diploma or equivalent is required; a bachelor's degree is preferred.
  • A valid Georgia Driver's License and the ability to travel frequently using a company vehicle.
  • Successful completion of a drug screening and background investigation.
  • Proficient in Microsoft Office applications, including Outlook and Excel, with a willingness to learn new software.

Work Schedule: 40 hours per week, Monday to Friday, 8 AM to 5 PM.
