Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and any appropriate ancillary medical personnel as needed.
Responsibilities- Conduct comprehensive home risk assessments, including medical history, diagnosis, and treatment, health education, and physician referrals.
- Work collaboratively with healthcare teams to provide high-quality patient care and achieve optimal health outcomes.
- Develop and implement individualized care plans to address patient needs and promote healthy behaviors.
- Provide education and support to patients and their families to promote health literacy and self-management.
- Collaborate with healthcare providers to ensure seamless care transitions and optimal patient outcomes.
- Master's degree in Nursing or related field required.
- Current RN and NP licensure in state of practice, including prescription authority.
- Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with electronic health records.
- Excellent verbal and written communication skills.
