We are seeking a highly skilled and compassionate Primary Care Physician to join our team at One Medical Seniors. As a Senior Health Physician, you will play a critical role in delivering high-quality, patient-centered care to our geriatric population.
Key Responsibilities- Provide comprehensive primary care to patients in an outpatient setting, with a focus on geriatric care.
- Partner with our care team to leverage coaching and integrated behavioral health models to engage patients in care and drive behavior change.
- Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population.
- Collaborate with our Medical Directors to drive practice innovation and improve patient outcomes.
- At least 2 years of primary care experience required.
- Currently licensed or ability to obtain licensure in the state of Arizona.
- The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups.
- Board Certification in Internal or Family Medicine required.
