Job Summary

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Brunswick Cove Living Center, LLC. As a Physical Therapist, you will play a critical role in helping our patients achieve their goals and improve their quality of life. You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs and goals of each patient.

Key Responsibilities
  • Assess and Evaluate Patients

Conduct thorough assessments of patients to identify their strengths, weaknesses, and goals. Develop and implement individualized care plans that address the unique needs and goals of each patient.

  • Develop and Implement Treatment Plans

Design and implement evidence-based treatment plans that promote optimal patient outcomes. Utilize a variety of treatment techniques, including exercise, education, and modalities, to help patients achieve their goals.

  • Provide Quality Patient Care

Deliver high-quality patient care that is personalized, compassionate, and respectful. Foster a positive and supportive environment that promotes patient engagement and empowerment.

  • Collaborate with Interdisciplinary Team

Work closely with our interdisciplinary team, including physicians, nurses, and other healthcare professionals, to develop and implement individualized care plans that address the unique needs and goals of each patient.

  • Document Patient Care Services

Accurately and thoroughly document patient care services, including assessments, treatment plans, and progress notes.

Requirements
  • Proven Work Experience

Proven work experience as a Physical Therapist, preferably in a long-term care setting.

  • Valid License

Valid license to practice physical therapy in the state of [insert state].

  • Excellent Interpersonal Skills

Excellent interpersonal skills, with the ability to communicate effectively with patients, families, and healthcare professionals.
