Job Summary

We are seeking a highly skilled and compassionate Hospice Nurse Practitioner to join our team at Home Healthcare and Hospice Community Services. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious illnesses, focusing on symptom management, pain control, and improving quality of life.

Responsibilities
  • Provide comprehensive assessments, develop, and implement individualized care plans in collaboration with the interdisciplinary team.
  • Manage symptoms, pain, and other complex medical conditions, ensuring timely and effective interventions.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Collaborate with the hospice medical director and other healthcare professionals to ensure high-quality, evidence-based care.
  • Participate in patient care conferences, IDT meetings, and other interdisciplinary team activities to ensure comprehensive care planning.
Requirements
  • Master's degree in Nursing or higher.
  • Licensed nurse practitioner in New Hampshire.
  • Certified as a Nurse Practitioner by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Authority to practice and prescribe as a nurse practitioner in the state where services are provided.
  • Medicare billing privileges as a nurse practitioner.
  • One year of experience as a nurse practitioner or as facsimile specialty experience as RN.
  • Knowledge of disease-specific local coverage determinant (LCD) criteria for hospice eligibility along with knowledge of CMS Face to Face regulatory requirements.
  • Knowledge of hospice philosophy and service components.
  • Current CPR certification.
