Job Summary

We are seeking a highly skilled Nurse Care Manager to join our team at Glades Talent. As a key member of our healthcare team, you will be responsible for managing chronic care and transition processes for high-risk patients.

Key Responsibilities:

  • Develop and implement comprehensive care plans in collaboration with a multidisciplinary care team.
  • Perform thorough assessments to identify and address physical, mental, and social health needs.
  • Ensure seamless transitions from acute to post-acute care settings.
  • Coordinate follow-ups to prevent readmissions and enhance patient care progression.
  • Utilize data to inform care decisions and participate in performance improvement initiatives.
  • Document all patient interactions in the Electronic Medical Record accurately and promptly.

Requirements:

  • Minimum 2 years of RN experience, preferably in case management or chronic care management.
  • Strong interpersonal, organizational, and communication skills.
  • Familiarity with chronic care management, Medicare systems, and performance improvement.
  • Bilingual (English/Spanish) preferred.
  • Bachelor's degree in nursing or healthcare-related field; Master's preferred.
  • Active RN License (Texas or Multi-state Compact).
