Job Summary

We are seeking a skilled and compassionate Per Diem Hospice LVN to join our team at The Elizabeth Hospice. As a Per Diem Hospice LVN, you will provide professional nursing care to patients in their homes or facilities, working under the supervision of an RN. This is a unique opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Assist RNs in analyzing patient situations and taking appropriate action
  • Evaluate patient responses to care and make adjustments as needed
  • Perform licensed nurse skills, including phlebotomy, as required
  • Implement and document teaching plans for patients and families
  • Provide support to agency staff and participate in audit and office duties as assigned
Requirements
  • Vocational Nurse Training
  • Minimum 12 months of experience as an LVN, with preferred experience in acute care, home health, or hospice
  • Current California LVN license
  • Familiarity with computerized bedside patient documentation
  • Ability to work as part of a team and take direction from others
  • Phlebotomy certification and bilingual skills are a plus
