Job Description

We are seeking a highly skilled Healthcare Leader - Primary Care Provider to join our team. As a key member of our healthcare team, you will be responsible for delivering high-quality patient-centered care, working collaboratively with physicians, registered nurses, and medical assistants to provide advanced practice physical and psychosocial assessments, diagnosis, and management of health and illness needs.

Key Responsibilities:

  • Provide high-quality patient-centered care, ensuring that each patient receives individualized attention and treatment.
  • Collaborate with healthcare professionals to develop and implement comprehensive care plans.
  • Conduct thorough physical and psychosocial assessments, diagnosing and managing various health conditions.
  • Prescribe and/or dispense medications as needed, maintaining accurate records and ensuring compliance with regulatory requirements.
  • Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures.

Requirements

  • Master's degree from an accredited Nurse Practitioner or Physician Assistant program required.
  • Current BLS certification, Registered Nurse License (if applicable), Nurse Practitioner or Physician Assistant License, DEA License, Board of Registered Nursing Certification of Nurse Practitioners, and Board of Registered Nurses Certification Furnishing Number required.

Preferred Qualifications:

  • Two plus years' experience as a Nurse Practitioner highly desirable; minimum of two years of experience in a community health setting preferred.
  • Bilingual (English/Spanish) language skills a plus.
