About the Role

We are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team at Phoenix Home Care and Hospice. As a Private Duty Nurse for Children and Young Adults, you will have the opportunity to make a profound impact on the lives of our patients and their families.

Key Responsibilities
  • Deliver high-quality, individualized nursing care to pediatric patients in their homes, based on their unique needs and treatment plans.
  • Assist in the development, review, and revision of patient care plans, ensuring that each patient receives the best possible care.
  • Build strong relationships with patients and their families, providing emotional support and reassurance in a compassionate and professional manner.
  • Monitor and record patient progress, reporting any changes in condition to the appropriate personnel and/or physician.
  • Provide ongoing education and supervision to patients, families, and other healthcare team members, as needed.
Requirements
  • Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license.
  • Excellent problem-solving skills and ability to multitask.
  • Strong communication and interpersonal skills, with a positive and professional attitude.
