Responsibilities:
- Provide one-on-one care to pediatric patients in their homes, focusing on their medical, emotional, and social needs.
- Develop and implement individualized care plans, collaborating with healthcare teams to ensure comprehensive care.
- Administer medications, perform medical procedures, and provide wound care as needed.
- Monitor patients' conditions, reporting any changes or concerns to healthcare teams.
- Collaborate with families and caregivers to educate them on patient care, medication management, and disease management.
- Participate in ongoing training and professional development to stay current with best practices and industry advancements.
Requirements:
- RN with a current valid license to practice as issued by the State Board of Nursing.
- Experience in pediatric care, preferably in home health or hospice settings.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Ability to work independently, with minimal supervision, and prioritize tasks to meet patient needs.
- Basic life support (BLS) certification and any other required certifications.
