Job Summary

We are seeking a skilled Licensed Practical/Vocational Nurse to join our team at Amergis. As a Clinic LPN, you will work in a remote clinic location to provide high-quality patient care under the supervision of a Registered Nurse.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care under the direct supervision of a Registered Nurse.
  • Identify and prioritize actions based on patient care requirements.
  • Perform interventions according to identified priorities, plan of care, and clinic policies and patient care outcome standards.
  • Provide a safe, comfortable, and therapeutic patient environment.
  • Initiate and evaluate patient and/or family education.
  • Document patient's response to nursing and other interventions.
  • Monitor the safety and effectiveness of the environment and equipment.
  • Participate in activities to facilitate good interpersonal communications with the chosen facility.
  • Administer vaccines or any necessary medications.
  • Make follow-up phone calls to patients.
  • Input information into the computer.
  • Help stock and maintain the cleanliness of the facility.
Requirements
  • Current Licensed Practical/Vocational Nurse License.
  • One year experience as an LPN/LVN preferred.
  • One year experience in a clinic setting preferred.
  • Complies with all relevant professional standards of practice.
  • Participation and completion of Amergis' Competency program when applicable.
  • Current CPR if applicable.
  • TB Questionnaire, PPD or chest x-ray if applicable.
  • Current Health certificate (per contract or state regulation).
  • Must meet all federal, state, and local requirements.
  • Successful completion of new hire training as applicable to job site.
  • Understand patient confidentiality and HIPAA requirements.
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required.
  • Computer proficiency required.
  • Must be at least 18 years of age.
