The Team Manager is a key leader in the Patient Care Team, responsible for overseeing the delivery of high-quality patient care services. This role requires strong leadership and management skills, as well as a deep understanding of the principles and practices of primary medical and nursing care.

Patient Care Services

  • Directs all patient care services, ensuring that services are rendered as integrated components of the interdisciplinary plan of care.
  • Reviews information on all pending patient admissions, new admissions, and discharges to ensure eligibility and appropriateness criteria are met.
  • Collaborates with the team physician and attending physician regarding palliative care measures for pain and symptom management.
  • Ensures complete and accurate data and documentation are available to make recertification decisions.
  • Reviews imminent discharges to ensure effective discharge planning.
  • Monitors the type and level of team services provided to ensure they are appropriate and responsive to patient/family needs.
  • Manages the team schedule and coordinates services 24/7 to all patients on the team.
  • Facilitates team meetings, ensuring interdisciplinary input into the update and revision of the plan of care.
  • Participates in on-call rotation.

Staff Supervision and Management

  • Interviews, selects, trains, supervises, evaluates, and dismisses team staff in conjunction with the Patient Care Administrator.
  • Ensures staff competence and performance levels through field visits, documentation evaluation, and team-based services.
  • Acts as a resource and mentor for staff on clinical issues, documentation, and customer service behavior.
  • Oversees staff and volunteer schedules, ensuring workload is distributed equitably.
  • Reviews and approves payroll, ensuring accurate time, mileage, and expenses.

Quality Improvement/Regulatory Compliance

  • Monitors clinical records and team activities to ensure accuracy, completeness, and compliance with VITAS policy and regulatory bodies.
  • Develops and implements performance improvement activities to respond to service issues and challenges.
  • Ensures team completes patient/family satisfaction surveys and implements Quality Improvement measures.
  • Assists with surveys and requests from regulatory bodies and intermediaries.
  • Performs substantive chart reviews to ensure quality care is being delivered.
  • Participates in Outcomes Management and annual program review.
  • Performs utilization review of continuous care and inpatient levels of care for all patients on the team.

Customer Service/Sales/Marketing

  • Ensures problems/grievances/service failures are addressed with team members, vendors, and other VITAS departments.
  • Analyzes customer service issues to identify causes and works with team members to improve performance.
  • Speaks with patients/families and attending physicians to resolve issues and prevent revocation.
  • Visits LTC and Contract Bed facilities to ensure care plan integration and customer satisfaction.
  • Participates in providing inservices to customers and other team members.
  • Participates in professional, voluntary, or community service organizations.

Cost Containment

  • Verifies/approves admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitors utilization of resources to ensure cost-effective delivery of services.
  • Controls and is accountable for productivity, labor, and patient care costs.
  • Approves all bills/invoices related to patient care services.

Professional Development

  • Attends inservices, educational seminars, and workshops.
  • Develops and achieves professional growth goals and objectives.
  • Participates as a mentor of newly hired Team Managers.

QUALIFICATIONS

  • Reliable transportation with appropriate license and insurance coverage.
  • Private telephone in home.
  • Knowledge of primary medical and nursing care, with a working knowledge of oncologic nursing and palliative care.
  • Basic understanding of interpersonal dynamics within an interdisciplinary health care team.
  • Working knowledge of VITAS information system (Vx) and management reports.
  • Two years successful supervisory experience or equivalent in a health care organization.

EDUCATION

  • Bachelor's degree preferred.
  • Current and valid R.N. License to practice in the state where the VITAS program is located.
