Overview

The Care Navigator plays a vital role in providing outreach, engagement, and support for adult clients at the Brien Center for Mental Health. This position is responsible for care coordination, comprehensive assessments, and the development of individualized treatment plans. The Care Navigator collaborates with clients to ensure seamless integration of services, facilitate care transitions, and enhance the delivery of treatment. Support is provided through ongoing evaluations and coordination with various programs and external agencies.

Key Responsibilities
  • Engage with adults enrolled in the program to provide outreach and support.
  • Identify and address cultural and linguistic needs to deliver tailored care.
  • Continuously assess and overcome obstacles to achieving goals in line with the Treatment Plan.
  • Facilitate therapeutic groups as necessary to support client needs.
  • Offer ongoing support to individuals post-treatment who require additional assistance.
  • Collaborate with the treatment team to create and execute effective treatment plans.
  • Monitor progress towards treatment goals and adjust plans as needed.
  • Assist clients in accessing care and ensure attendance at appointments.
  • Provide transportation assistance for necessary appointments when required.
  • Guide individuals in navigating community resources and services.
  • Support safe transitions of care across different service settings.
  • Ensure effective communication between clients and healthcare providers.
  • Participate in required meetings and training sessions.
  • Maintain accurate client records and documentation in a timely manner.
Additional Duties
  • Engage in regular supervision sessions.
  • Attend mandatory agency training programs.
Qualifications
  • Bachelor's degree in a relevant Human Services field is required.
  • Experience in community-based behavioral health support is preferred.
  • Familiarity with local resources and treatment systems is advantageous.
  • A valid driver's license and reliable transportation are necessary.
