Job Summary:

The Ambulatory Clinical Pharmacist will be responsible for providing on-site integrated support to patients and physicians, ensuring the provision of safe, efficacious, and cost-effective drug therapy for all Oncology patients. This role requires a strong clinical knowledge of pharmaceuticals and disease states, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Provide face-to-face and one-on-one phone-based disease management with patients
  • Review prior authorization requests for medications requested under the pharmacy benefit
  • Collaborate with healthcare professionals to ensure seamless care coordination
  • Monitor and review medication regimens, advising clinicians on evidence-based therapy as indicated
  • Develop and implement clinical criteria and medical policies for the review of medications
Requirements:
  • Doctor of Pharmacy (PharmD) degree
  • Licensed Pharmacist in the state of employment
  • 1+ years' experience as a clinical pharmacist with completion of PGY1 residency
  • Completion of PGY2 residency specializing in oncology required
