Job Summary:

As a Registered Nurse (RN) Onsite Assessor for Wellcove, you will be responsible for conducting Long Term Care Insurance eligibility assessments in the comfort of the client's home or facility. This is a non-medical, observational assessment that requires you to document client demographics, physician information, and functional and cognitive levels of independence.

Key Responsibilities:
  • Administer cognitive screenings (MMSE or SPMSQ) and document client ability to complete ADL and IADL tasks
  • Document client demographics, physician information, and medication and diagnosis lists
  • Complete assessments within 90 minutes (about 1.5 hours)
  • Submit completed assessment documentation within 24 hours of the scheduled appointment
  • Remain available for clarifications and questions from the Clinical Review team

Requirements:
  • Current unencumbered RN licensure in state of practice
  • Must have or be willing to obtain a professional liability insurance policy
  • Must have a current driver's license, vehicle insurance, and access to dependable vehicle and/or public transportation
  • Minimum of 2 years of home health experience or nursing in an equivalent area of practice serving geriatric clients
  • Must have own computer/tablet and internet access

Working Conditions:
  • Assessments are performed in the client's home or facility setting
  • Assessor's are required to wear a mask and maintain social distancing, if asked
  • Client/Family or Facility may have additional COVID-19 requirements
