Job Description

At Monarch Communities, we are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Reflections Program Coordinator LPN. This is a unique opportunity to make a meaningful difference in the lives of our residents and their families.

Key Responsibilities:

  • Coordinate the recruitment and hiring process for the Reflections Care Management team members
  • Complete staff performance appraisals and disciplinary actions
  • Facilitate general orientation for all staff on the Reflections Program
  • Conduct monthly in-services on various dementia-related topics for all staff
  • Responsible for environmental rounds to ensure the safety of exit doors, resident room safety, and unit safety, and adherence to brand standards
  • Provide direct support to care management staff for behavior symptom management and resident care approaches
  • Coordinate and facilitate the RCP Training program for primary staff on the unit, responsible for tracking completion and scheduling of RCP classes
  • Suggest residents for restorative potential and ensure the program is consistently carried out by staff to maintain residents' maximum practicable level of function
  • Cooperate with other resident services for complete evaluation of care
  • Assist in establishing linkages with and arranging for services from public and private sources for income, health, mental health, and social services
  • Assist residents in making applications for and maintaining income entitlements and public benefits
  • Coordinate care planning meetings per state regulations
  • Coordinate, implement, and monitor all resident service plans with the Wellness Director
  • Utilize Brandywine standards and standards of professional organizations in conducting resident care
  • Demonstrate knowledge and skills necessary to safely provide care appropriate to the age of residents managed on the unit
  • Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained
  • Assist with admits, transfers, and discharges of residents as required
  • Report to the RN to assess and document resident and family response to interventions
  • Provide written and/or oral status reports of residents to ensure necessary follow-up actions
  • Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures
  • Assist the physician on rounds and ensure that pertinent information is communicated to and from him/her
  • Assist in resident rehabilitation activities, where required, assist in ambulation, and provide emotional support, monitor the results of physical therapy activities
  • Give direct physical and psychological care in emergencies, as well as in the presence of illness or disability, to maintain life, provide comfort, reduce stress, and enhance the resident's ability to cope, report all observations and changes in condition to the RN
  • Prepare and administer medications as ordered by the physician, utilizing all standards/policies of medication administration
  • Ensure that adequate supplies and equipment are on hand to meet the nursing needs of the residents
  • Comply with Controlled Substances policy and procedures in maintaining accurate inventories and controlled logs, follow appropriate guidelines for investigation of discrepancies
  • Review medication administration records for completeness of information, accuracy in the ECP of the physician's order, and adherence to stop order policies, report any discrepancy to the RN
  • Dispose of drug and controlled substances as required, and in accordance with established procedures
  • Monitor residents, report to the RN, and take appropriate action as instructed by the RN and MD for side effects and adverse reactions to medications
  • Adhere to all infection control policy and procedures
  • Administer and document all treatments as ordered and in accordance with policies and procedures
  • Maintain accurate resident care records and document pertinent data reflecting the use of the nursing process, the care provided, and resident response to care, report all changes in resident care or condition to the RN
  • Assist in the development of resident care assignment sheets
  • Review all resident care manager assignments for completion
  • Work in partnership with the INVEST Coordinator to ensure a therapeutic and socially engaging environment
  • Ensure the safety of the unit through environmental rounds

Requirements:

  • Licensed Practical Nurse (LPN) with a valid license
  • Proof of successful graduation from an accredited Nursing Program
  • Previous experience with dementia populations preferred
  • Previous experience in a hospital, assisted living, skilled nursing care, or other related medical community required

  • Moves intermittently during working hours and subject to frequent interruptions
  • May be subject to hostile and emotionally upset residents, family members, personnel, and visitors
