Job Summary

We are seeking a highly skilled Physician Assistant or Nurse Practitioner to join our team at Yale New Haven Health. As a key member of our multidisciplinary healthcare team, you will provide exceptional patient care and work collaboratively with our attending physicians to deliver high-quality, patient-centered care.

Responsibilities
  • Provide comprehensive patient assessments, triage, and therapeutic management of patient/family needs throughout the disease process.
  • Work collaboratively with attending physicians to develop and implement patient treatment plans, including ordering diagnostic tests and prescribing medications as needed.
  • Provide patient education and psychosocial support to patients and their families, promoting a patient-centered environment that addresses physical, psychosocial, spiritual, and functional needs.
  • Ensure accurate and timely documentation of patient information, adhering to hospital and regulatory requirements.
  • Participate in quality management initiatives, demonstrating a commitment to excellence and patient safety.
Qualifications
  • Graduate from an accredited Nurse Practitioner or Physician Assistant Program.
  • Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA) or Connecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification, as appropriate for position profile.
  • Experience in a clinical setting, preferably in the given specialty/service line, is preferred but not required.
