Position Overview: The Patient Care Continuity Specialist collaborates closely with the treatment team to formulate, coordinate, and communicate comprehensive care plans tailored to each patient.
Key Responsibilities:
- Maintain an updated network of referrals.
- Engage with patients to assess their discharge planning requirements as identified by the treatment team.
- Facilitate effective communication between the treatment team and various referral and continuing care resources.
- Document care plans, communications, and meetings meticulously.
Qualifications:
- Bachelor's Degree in Behavioral Science or a related discipline is preferred.
- A minimum of one year of experience in a behavioral health care setting is advantageous.
- Strong understanding of and compliance with governmental, regulatory, and organizational policies.
- Proficient in maintaining confidentiality, managing multiple tasks, coordinating projects, and accurately recording meeting minutes.
Work Environment: This role involves exposure to various risks, including potential interactions with aggressive individuals and critical incidents. We are a Drug-Free Company.
