We're seeking a skilled and compassionate professional to conduct in-home assessments, perform evidence-based practice screenings, and identify diagnoses to inform care management. Your expertise in gerontology, cardiology, internal medicine, or endocrinology will be invaluable in providing high-quality patient care.
Key Responsibilities:- Conduct comprehensive physical exams and vital sign assessments
- Perform point-of-care testing and evidence-based practice screenings
- Identify diagnoses and develop care plans
- Communicate findings with patients' primary care physicians
- Intervene in urgent and emergent situations
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
- Ability to practice autonomously or without formal supervision
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
