We're seeking a skilled and compassionate professional to conduct in-home assessments, perform evidence-based practice screenings, and identify diagnoses to inform care management. Your expertise in gerontology, cardiology, internal medicine, or endocrinology will be invaluable in providing high-quality patient care.

Key Responsibilities:
  • Conduct comprehensive physical exams and vital sign assessments
  • Perform point-of-care testing and evidence-based practice screenings
  • Identify diagnoses and develop care plans
  • Communicate findings with patients' primary care physicians
  • Intervene in urgent and emergent situations
Requirements:
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification
  • Ability to practice autonomously or without formal supervision
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
