Job Title: Advanced Practice Provider - Nurse Practitioner or Physician Assistant

We are seeking a highly skilled Advanced Practice Provider - Nurse Practitioner or Physician Assistant to join our team at LiveWell Choice, LLC. As a key member of our clinical services team, you will be responsible for providing high-quality, patient-centered care to our long-term care residents.

Job Summary:

The Advanced Practice Provider will work collaboratively with our interdisciplinary team to deliver comprehensive, value-based care to our members. This includes conducting comprehensive assessments, developing and implementing individualized care plans, and providing ongoing medical management and support to our residents.

Key Responsibilities:
  • Provide primary care and services to a panel of Longevity Health members, including treating in the most appropriate setting based on the member's goals of care.
  • Execute and deliver Longevity Health Model of Care activities, treating in the most appropriate setting, and developing and implementing advanced care planning.
  • Facilitate and administer Health Plan clinical operations functions, including prior authorization, Part A and B services, medication reviews, HEDIS and STARs quality gap closure, and promoting membership growth.
  • Serve as a trusted advisor to members, loved ones, and nursing home staff, and member advocate, educating care teams on member needs and early identification of changes in condition.
  • Deliver effective and frequent communication and coordination with members' PCPs, loved ones, and care team members on members' care needs, goals of care, treatment plans/changes.
Requirements:
  • State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required.
  • Will allow acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physician's Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
  • DEA License required.
  • Must have an active national provider number (NPI) and an active Medicare provider number.
  • Must be eligible to participate in the Medicare program as a provider and registered.
  • Valid state driver's license with a good driving record and proof of automobile insurance required.
Education and Training:
  • Master of Science in Nursing as a Nurse Practitioner from an nationally accredited program OR Master's Degree from an nationally accredited program.
Knowledge and Experience:
  • Minimum of 6 months experience as a Nurse Practitioner/Physician's Assistant AND has some experience with geriatric/hospice or palliative care population prior to becoming an NP/PA OR is a licensed and board-certified new graduate NP/PA supervised by an experienced NP/PA in the LHP New Graduate Fellowship program for 6 months and successfully completes all competencies for independent practice as an LHP APP.
  • Experience working in the nursing home environment is strongly desired.
  • Computer/Technology proficiency required.
  • The ability to engage and communicate effectively with physicians, patients, family members - excellent people skills.
  • Self-directed but functions well in a team environment.
  • Excellent clinical skills - assessment, diagnosis, treatment, prescribing specifically for the chronically ill population.
  • Can function independently, demonstrated critical thinking skills, organization, and prioritization skills.
