Primary Care Clinic Nurse Practitioner

We are seeking a highly skilled and experienced Nurse Practitioner to join our team at the Ventura County Health Care Agency. As a Primary Care Clinic Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to our patients.
Job Summary:The Primary Care Clinic Nurse Practitioner will work under the supervision of a physician and follow written guidelines of protocol to perform medical examinations, selected diagnostic and therapeutic tasks, and other special procedures to assist in the delivery of primary care to patients.
Responsibilities:- Provides complete, comprehensive, family-oriented, culturally-sensitive clinical care for patients throughout the life cycle.
- Prescribes medications and treatment, monitors patients' progress, and adapts treatment as required.
- Inoculates and vaccinates patients to immunize patients from communicable diseases.
- Advises patients concerning evidence-based lifestyle changes that improve patient health and works with patients to develop self-management goals.
- Coordinates with care team to provide high-quality patient care and ensure patient treatment goals are met.
- Possession of a Master's or higher degree in nursing or a clinical field related to nursing.
- Six (6) months of full-time Nurse Practitioner experience (preferably in the area of clinical assignment).
- Current valid license as a Registered Nurse issued by the State of California Board of Registered Nursing.
- Current valid Nurse Practitioner Certificate issued by the State of California Board of Registered Nursing.
- Valid, active DEA license number.
- National Provider Identifier (NPI) number.
The Primary Care Clinic Nurse Practitioner will work in a fast-paced, dynamic environment with a high volume of patients. The ideal candidate will be able to work effectively in a team setting and communicate effectively with patients, families, and healthcare staff.
