About the JobThis role offers the opportunity to work in a supportive and collaborative environment where you will have the chance to make a real difference in the lives of our patients. As an LPN, you will be responsible for delivering direct patient care, including administering medications, monitoring vital signs, and performing various treatment procedures.
You will also be expected to document patient information, communicate with families and caregivers, and participate in quality improvement initiatives. As a member of our team, you will have the opportunity to work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Patient Care & Service:
- Promote and restore patients' health by completing the nursing process;
- Collaborate with physicians and multidisciplinary team members;
- Perform various treatment procedures;
- Provide physical, educational, and emotional support to patients, friends, and families;
- Supervise assigned team members;
- Documentation:
- Ensure concise, pertinent, and complete resident care documentation using computerized medical record process;
- Supports CNA team:
- Direct the CNA in the timely completion of quality individualized personal care;
- Ensure CNA documentation is accurate and complete;
To be successful in this role, you must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You should be a licensed practical nurse in good standing with the New York State Education Department and possess current Basic Life Support certification from the American Heart Association.
In addition to your clinical expertise, you should demonstrate a commitment to delivering high-quality patient care and a passion for working collaboratively with others to achieve positive outcomes.
