As a key member of our HouseCalls team, you'll play a vital role in providing annual clinical assessments to patients in the comfort of their own homes. This proactive approach helps identify and mitigate health risks, ensuring patients receive the care they need to thrive.
**Key Responsibilities:**
- Conduct in-home assessments, including a comprehensive review of medical history, vital signs, and physical examination
- Perform evidence-based practice screenings, including point-of-care testing as needed
- Identify diagnoses and develop care management plans to address treatment gaps
- Communicate findings with patients' primary care physicians to ensure coordinated care
- Recognize and respond to urgent and emergent situations, providing timely interventions
- Educate patients on disease management, medication adherence, and healthy lifestyle choices
Requirements:
- Active NP license or ability to obtain by start date, with no encumbrances or disciplinary actions
- APCs must have approval to practice autonomously or without formal supervision, if applicable
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain certification by start date
- Ability to complete physical demands of the job, including lifting, navigating stairs, and performing physical assessments
- Access to reliable transportation for travel up to 85% within licensed states
Preferred Qualifications:
- 1+ years of clinical experience in the highest level of education, clinical setting
- Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology
- Home health care or home visit experience
- May be required to obtain additional licensure in other geographic areas
