About the Role

This full-time position offers a competitive salary range of $65,000 - $85,000 per year, depending on experience. As a Case Manager, you will play a vital role in coordinating patient care, working closely with healthcare teams, and providing emotional support to patients and their loved ones.

Key Responsibilities
  • Conduct thorough assessments of patients' physical, psychological, and social needs
  • Develop and implement personalized care plans that address patients' goals and preferences
  • Coordinate with healthcare teams to ensure seamless care delivery
  • Provide emotional support and guidance to patients and their families
  • Maintain accurate and up-to-date patient records
Requirements

To be successful in this role, you will need:

  • A current license to practice as a Registered Nurse in the state of Texas
  • Experience in hospice care or a related field
  • Excellent communication and interpersonal skills
  • The ability to work collaboratively as part of a multidisciplinary team
  • A strong commitment to delivering high-quality patient care
