Job Summary:

The Home Health Licensed Vocational Nurse will be responsible for providing high-quality nursing care to patients in their homes, assessing their health needs, and developing personalized care plans to meet their unique requirements.

Key Responsibilities:
  • Assess patient health problems and needs, and implement nursing care plans to address them.
  • Provide skilled nursing care under the direct supervision of the Clinical Supervisor, including vital sign monitoring and medication reviews.
  • Coordinate with physicians, therapists, and other healthcare professionals to ensure comprehensive care.
  • Maintain accurate and complete documentation of patient care, including medical records and observations.
  • Work collaboratively with the healthcare team to provide education and support to patients and their families.
  • Monitor patient conditions, record symptoms, and report changes to the healthcare team.
  • Modify patient treatment plans as needed based on patient responses and conditions.
Requirements:
  • Associate degree in nursing required; Bachelor's degree preferred.
  • Current and valid California Licensed Vocational Nurse license.
  • One year of LPN experience and current CPR certification required.
  • Strong interpersonal and communication skills, with the ability to build rapport and establish trust with patients.
  • Computer literacy and strong organizational skills.
  • Ability to meet physical demands of the job, including lifting up to 25 pounds.
Preferred Qualifications:
  • Home health care experience.
  • Bilingual (Spanish) preferred.
