About the RoleAs a Clinical Care Specialist - Home Health, you will provide direct patient care in the patient's home under the supervision of a physician. Your responsibilities will include following start of care plans, providing treatment, discharges, wound care, medication administration, and collaborating with a clinical team to achieve positive patient care outcomes.
Essential Functions- Manage and coordinate patient care plans, including case management and follow-up skilled nursing visits.
- Develop and implement treatment plans based on physician orders and initial patient assessments.
- Submit regular patient care progress reports to attending physicians and clinical staff.
- Discharge patients after consultation with physicians and clinical managers.
- Provide nursing care and written care plans.
- Participate in and monitor patient and family care compliance activities.
- Manage and coordinate patient care resources, including PT, OT, Hospice, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults.
- Minimum of Associate degree (or higher) in Nursing.
- Current and unrestricted California Registered Nurse license.
- Current CPR/BLS certification.
- Minimum of 1 year RN experience in an institutional setting, including Acute Care, Rehab, Hospital, Home Health, and Hospice.
- Experience in wound care, patient teaching, medication management, lab draws, and IV infusion is a plus.
- Ability to chart with Electronic Medical Record Software, such as Wellsky or Kinnser, is a plus.

We offer a $5,000 sign-on bonus for full-time RNs.
