**Job Summary:**
We're seeking a compassionate and skilled LPN to join our team in the Ellenville/Napanoch area. As a Home Health LPN, you'll be responsible for providing technical skilled care to patients in their homes, working closely with our interdisciplinary team to develop and implement personalized care plans.

**Key Responsibilities:**

* Provide quality patient care and perform technical skilled care in the patient's home
* Instruct patients and their families in the care of the patient and maintenance of a healthy environment
* Observe, record, and report to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily
* Collaborate with the healthcare team to ensure seamless care coordination

**Requirements:**

* Current LPN licensure in the state of practice
* Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
* CPR Certification required
