Family Support Specialist

Position Overview:

Eliot is in search of a Family Partner who possesses personal experience as a caregiver for a child or youth with special needs, particularly those related to mental health. The ideal candidate will have a background in navigating child and family service systems and will be adept at educating families engaged in these systems.

The role requires a compassionate approach to communicate with other parents in a supportive and non-judgmental way regarding their unique needs.


Our Services:

Our Child, Youth & Family Behavioral Health Services integrate various service systems, including:

  • Intensive Care Coordination
  • Family Support & Training
  • In-Home Therapy
  • Therapeutic Mentoring
  • Outpatient Counseling & Psychiatry
  • Additional community-based programming

Service delivery is collaborative, family-driven, and youth-guided, focusing on the strengths, needs, and preferences of individuals and families.


Key Responsibilities:


1. Engage parents/caregivers in home and community activities aimed at addressing goals outlined in the youth's treatment plan or Individualized Care Plan (ICP).

2. Assist parents/caregivers in fulfilling the needs of the youth through education, support, coaching, modeling, and guidance.

3. Educate parents/caregivers on how to connect with community resources and treatment providers.

4. Support parents/caregivers in advocating for necessary services and resources to meet the youth's needs.

5. Guide and empower parents/caregivers, including connecting them with individual, peer/parent support, and self-help groups.

6. Participate in Care Planning Team (CPT) meetings and contribute to discharge planning.

7. Collaborate with providers and attend all relevant meetings, including IEP, hospital discharge, and treatment team meetings.

8. Provide telephone support and outreach to parents/caregivers.

9. Maintain regular contact with collateral providers through phone and face-to-face interactions.

10. Offer transportation to parents/caregivers as necessary.
