About the Role:

We are seeking a skilled Home Health Physical Therapy Assistant to join our team. As a member of our care team, you will be responsible for providing top-notch care and support to patients in their homes.

Key Responsibilities:

  • Under the supervision of a Physical Therapist, provide hands-on treatment and instruction to patients, focusing on improving their well-being and enhancing their quality of life.
  • Develop and implement individualized plans of care, taking into account each patient's unique needs and goals.
  • Collaborate with other healthcare professionals to ensure comprehensive care and seamless coordination of services.
  • Conduct thorough assessments and evaluations to identify areas for improvement and develop targeted interventions.
  • Maintain accurate and detailed records of patient progress and outcomes.

Requirements:

  • A current license to assist in the practice of physical therapy in the state of practice.
  • Current CPR certification.
  • Reliable transportation and a valid driver's license.
