Job Summary

We are seeking a highly skilled and compassionate Wound Care Certified Advanced Practice Provider to join our team at Resilient Healthcare. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals with complex wound care needs.

Key Responsibilities:

  1. Conduct thorough assessments of patients' wound care needs, developing and implementing individualized care plans to promote optimal healing and prevent complications.
  2. Provide expert wound care services, including debridement, I&Ds, and biopsies, as indicated.
  3. Collaborate with referring physicians and other healthcare professionals to coordinate care and ensure seamless transitions.
  4. Interpret diagnostic test results and make recommendations for further testing or treatment as needed.
  5. Prescribe medications, equipment, and supplies to facilitate wound healing and prevention.
  6. Lead support groups and health promotional activities to educate patients and families on wound care and related topics.
  7. Maintain accurate and comprehensive patient records, tracking progress and outcomes.
  8. Participate in quality improvement initiatives and contribute to the development of policies and procedures affecting patient care.

Requirements:

  • Master's degree in Science of Nursing or Physician Assistant Studies or related field.
  • Minimum 3 years of advanced practice provider experience.
  • Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas.
  • Family Nurse Practitioner or Adult Gerontology or Acute Care Nurse Practitioner certification or Physician Assistant Certification from National Commission on Certification of Physician Assistants.
  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BLCLS) and Certification.
  • American Red Cross or American Heart Association Advanced Life Support Course (ALS or ACLS) and Certification.
  • Satisfactory references from employers and/or professional peers.
  • Satisfactory employment verification.
  • Satisfactory licensing verification.
  • Satisfactory education verification.
  • Satisfactory criminal background check.
  • Satisfactory drug screen.

Working Conditions:

This role operates in a patient's home medical office setting, requiring travel by automobile and exposure to various environmental conditions, bloodborne pathogens, physical hazards, and chemical hazards.

Equipment Used:

This role routinely uses standard office equipment, medical equipment, and requires the ability to navigate technology for telehealth visits.

Physical Requirements:

This role requires regular standing, walking, reaching, twisting, stooping, and kneeling, as well as the ability to use hands and fingers to handle or feel, and to talk or hear.
