Key Responsibilities:
  • Provide medical case management services to individuals through in-person and telephonic communications with patients, physicians, and other healthcare providers.
  • Utilize your medical and nursing knowledge to discuss treatment plans with physicians and explore alternative options.
  • Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
  • Assess, plan, implement, and evaluate patient progress.
  • Attend doctors' visits, hospital discharges, and other relevant meetings to ensure effective care.
  • Implement care strategies, including negotiation of durable medical equipment and nursing services.
  • Travel locally (up to 60% of the workweek) and occasionally overnight to meet with patients and healthcare providers.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current RN licensure in the state of operation.
  • Certification as a CCM, CIRS, or other case management certifications is preferred.
  • A valid driver's license, reliable transportation, and ability to travel to assigned locations is required.
