Job Summary:

We are seeking a skilled Registered Nurse to join our Adult Day Health team at Sevita. As a Nurse Care Specialist, you will play a vital role in providing high-quality care to our patients with complex medical needs.

Key Responsibilities:

  • Review and update patient medical records to ensure accurate and up-to-date information.
  • Provide treatment to patients under the direction of a physician, ensuring the highest level of care and attention to detail.
  • Assist patients with obtaining prescribed medication, promoting adherence to treatment plans.
  • Measure and record patient's temperature, vital signs, and other health metrics to monitor their condition.
  • Triage patients who are ill, injured, or experiencing behavioral issues, providing a safe and supportive environment for all patients.
  • Document patient care activities in the patient's medical record, maintaining accurate and detailed records.
  • Complete administrative tasks, including scheduling appointments, preparing reports, and maintaining patient records.
  • Stay up-to-date with the latest medical research, best practices, and industry developments to provide the best possible care.

Requirements:

  • Graduate of an accredited RN program, with a strong foundation in nursing principles and practices.
  • One year of experience working with medically complex individuals, with a proven track record of providing high-quality care.
  • Current state RN License, with a valid driver's license, car registration, and auto insurance (if providing transportation to patients).
  • CPR/First Aid certification, with a commitment to ongoing professional development and training.
