NP Team Lead - Mental Health Professional

At ViaQuest Psychiatric & Behavioral Solutions, we offer unique and individualized care to the aging population, those with developmental disabilities, and those suffering from extreme trauma. Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers, and psychiatrists to ensure the best outcomes for those we serve.

Responsibilities:
  • Ensure treatment plans are up-to-date for each client as appropriate.
  • Coordinate individual plans of individuals served, as applicable.
  • Provide assessment services effectively and efficiently.
  • Complete client screenings timely as assigned.
  • Complete comprehensive and succinct progress notes for each client.
  • Ensure the person served is oriented to their services.
  • Share information on how to access community resources relevant to client needs.
  • Communicate progress of the person served to the appropriate persons.
  • Facilitate the transition process, including arrangements for follow-up services.
  • Involving the family or legal guardian when applicable or permitted.
  • Provide trainings as requested by manager for internal and external customers, as well as other coworkers.
  • Obtain clinical supervision from collaborating Physician, as necessary.
  • Identify and address gaps in service provision.
  • Participate in management team meetings and other meetings as assigned.
Requirements for this position include:
  • Graduate of an accredited Nurse Practitioner program.
  • PMHNP license required.
  • Experience working as a PMHNP is required.
  • Supervisory experience may be preferred.
  • Outpatient and tele-psychiatry experience is preferred.
  • Willingness to work with a variety of ages, including children and adolescents.
  • Willingness to travel throughout assigned service area.
