Job Summary

Eskenazi Health seeks a highly skilled Physical Therapist II to join our team. As a key member of our rehabilitation department, you will be responsible for providing exceptional physical therapy services to patients, while also serving as a resource and mentor to colleagues.

Key Responsibilities
  • Evaluation and Treatment
    • Conduct comprehensive evaluations of patients to identify treatment needs and develop personalized plans of care.
    • Administer a range of standardized and non-standardized assessments to inform treatment decisions.
    • Develop and implement creative and effective treatment plans that address the unique needs of each patient.
  • Documentation and Productivity
    • Accurately and timely document patient evaluations and treatment plans in accordance with hospital standards.
    • Meet or exceed productivity standards, while prioritizing patient care and ensuring efficient use of time.
  • Professional Communication and Collaboration
    • Effectively communicate with patients, families, and healthcare teams to ensure seamless care coordination.
    • Collaborate with colleagues to share knowledge, expertise, and best practices in physical therapy.
  • Performance Improvement and Student Education
    • Participate in quality improvement initiatives to enhance patient outcomes and departmental efficiency.
    • Provide mentorship and education to students and new staff members, promoting successful onboarding and professional growth.
Requirements
  • Education and Licensure
    • Graduate of an American Physical Therapy Association (APTA)-accredited physical therapy program.
    • Licensed as a Physical Therapist in the State of Indiana or eligible for licensure.
  • Experience
    • Minimum of two years of clinical experience in a setting similar to Eskenazi Health.
