The SRO Residential Services Coordinator 3 (RSC 3) plays a crucial role in providing direct support, advocacy, and education to residents. Compensation for this role ranges from $22 to $22.30 per hour.


Key Responsibilities:


- Collaborate with management to provide prospective residents with tours and input on admissions.

- Ensure comprehensive service planning through a person-centered approach during residents' engagement with the program, including recovery-oriented services and ongoing discharge planning.

- Conduct quarterly service plan reviews with residents and their support teams to assess progress and make necessary updates.

- Deliver teaching, monitoring, and recovery-oriented support in accordance with OMH 595, covering various rehabilitative services such as Assertiveness Training, Community Integration, Daily Living Skills, Health Education, Medication Management, Parenting Training, Rehabilitative Counseling, Skill Development, Socialization, Substance Abuse Services, and Symptom Management.

- Observe, document, and report resident behaviors and progress in line with agency standards.

- Maintain accurate resident files, including progress notes, service plan updates, and relevant medical records.

- Schedule annual appointments for physicals, eye exams, and other medical needs, ensuring transportation is arranged when necessary.

- Collaborate with the Resident's Health Home Care Manager and community service providers to ensure continuity of care.

- Act as a liaison between residents and their clinical providers regarding mental health monitoring.

- Assist residents with daily living skills, including room upkeep, meal preparation, and laundry assistance.

- Support residents in fostering healthy connections with family and community, while encouraging the development of new interests.

- Coordinate and participate in recreational activities for residents as needed.

- Assist Medication Coordinators with medication supervision and dining room responsibilities during assigned shifts.

- Ensure safe transportation of residents when required.

- Uphold resident confidentiality and demonstrate a caring attitude towards all individuals involved.

- Adhere to safety regulations for self, residents, and staff.

- Attend required staff meetings and training sessions.

- Collaborate as a member of the residential team, providing relevant input for treatment meetings and reviews.

- Perform additional duties as necessary for the effective operation of the program and the well-being of residents.


Qualifications:

Education and Experience:
Candidates must meet one of the following criteria:

A. Bachelor's degree in a Human Services field with at least one (1) year of relevant experience.
B. Associate's degree in a Human Services field with a minimum of three (3) years of related experience.
C. High School diploma/GED with at least five (5) years of experience in a Human Services field.

*A clean and valid New York State driving record is required, as outlined in DePaul's personnel policy. Candidates must be at least eighteen (18) years of age.
