About the Role:
We are seeking a compassionate and skilled Care Manager to join our team. In this role, you will be responsible for delivering high-quality care and services to a consistent group of residents and their families. Your primary focus will be on building meaningful relationships with our residents, understanding their unique needs and preferences, and providing individualized support to enhance their quality of life.
Responsibilities:
- Build Meaningful Relationships: Develop strong bonds with your assigned residents and their families, providing emotional support, guidance, and advocacy as needed.
- Care Planning and Coordination: Collaborate with interdisciplinary teams to create and implement individualized care plans that cater to the unique needs and preferences of each resident.
- Life Enrichment Activities: Participate in the development and implementation of life-enriching activities, programs, and events that promote socialization, engagement, and enjoyment among residents.
- Communication and Collaboration: Maintain effective communication with residents, families, healthcare professionals, and internal teams to ensure seamless care coordination and service delivery.
- Continuous Learning and Development: Stay up-to-date with industry best practices, attend training sessions, and participate in professional development opportunities to enhance your skills and knowledge.
Requirements:
- Dedication to Serving Seniors: A genuine passion for working with seniors, excellent customer service skills, and a commitment to delivering exceptional care and services.
- Education and Experience: High school diploma or equivalent required; previous experience working with seniors preferred.
- Skills and Qualifications: Strong organizational, time management, and problem-solving skills; ability to work effectively in a team environment; basic computer skills and proficiency in electronic medical records (EMRs).
- Certifications and Licensures: Valid certifications and licensures as required by state regulations.
