Job Title: Home Care Coordinator

The estimated annual salary for this position is around $55,000-$65,000, depending on experience and qualifications.

Job Summary: Kintegra Health seeks a dedicated and compassionate Home Care Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in developing and implementing home care services for our frail elderly population. Your expertise will ensure that our participants receive high-quality care in the comfort of their own homes.

Key Responsibilities:
  1. Assess the home care needs of our participants using the nursing process and develop specific plans of care.
  2. Coordinate 24-hour care delivery and implement all home care services, including personal care services.
  3. Authorize and manage Durable Medical Equipment (DME) and home supplies.
  4. Reconcile invoices for personal care service hours and home supplies.
  5. Perform acute, in-home visits as requested by primary care providers and supervisors.
